  1. Role of  Transbronchial Cryo-biopsy & Video-assisted thoracoscopic (VATS) lung biopsy in diagnosis of ILD • Transbronchial lung cryo-biopsy should be considered as an alternative to SLB: in carefully selected patients at specialized centers • SLB: diagnosis of ILD if the HRCT does not show a characteristic pattern of a specific ILD sub-type • VATS lung biopsy should be preferred over open lung biopsy (OLB) • Patients should preferably have a FVC≥55% and DLCO≥35% and the presence of no or mild pulmonary hypertension

  2. Cryo lung biopsy from right lower lobe • 45 Y M • Difficulty in breathing and cough since 4y June 2019 Second shows peribronchovascular airway infilterates interstitial aggregate of lymphocytes with fibrosis and pigment aggregate → Chronic HP

  3. Role of integrated multidisciplinary team (MDT) in diagnosis of ILD MDT includes • Pulmonologist • Radiologist • Pathologist • Rheumatologist (optional)
