Management of Suspected Transfusion Reactions
In the event of a suspected transfusion reaction, stabilize the patient immediately and notify the attending physician. Complete the clerical check and fill out the Transfusion Reaction form, then contact the Blood Bank. If the patient exhibits only itching and hives, other symptoms should resolve before resuming the transfusion, ideally after administering diphenhydramine. For further evaluation, draw two purple top tubes and send them to the Blood Bank with the remainder of the blood unit. If hemolysis symptoms are present, send a urine sample for analysis. Await pathology approval before transfusing another unit, unless urgent.
