Aims • To develop a ‘big picture’ of the main pathways to achieving the high level outcomes identified for older people • To bring together and summarise the existing relevant evidence and indicators for older people • To strengthen existing mechanisms for planning and evaluating services, whether through joint commissioning or community planning • To understand the contribution of partnership activities to delivering outcomes

Fitting with outcomes for integrated care - Discussion • Is the strategic model helpful for future planning and monitoring impact of local services? • What are the key challenges to deliver local targets while attempting to align local priorities with H and SC integration outcomes and the proposed strategic model/vision for older people?
