Discharged on Antithrombotic Therapy • Patients with an active problem of Ischemic Stroke that are not discharged on an antithrombotic will trigger this pop-up on the med rec. • You will see the pop-up below and will either need to order an antithrombotic or select a reason to override.
Anticoagulation for A-fib/ Flutter • Patients with an active problem of A-Fib/ Flutter that are not discharged on anticoagulation therapy will trigger this pop up on the med rec. • You will see the pop-up below and must either order anticoagulation or select an override reason.
Antithrombotic Therapy by end of day 2 • Patients with an active problem of Ischemic Stroke when no antithrombotic is ordered on admission med rec will trigger this pop up. • You will see the pop-up below and must either order an antithrombotic or select an override reason.
Discharged on Statin • Patient with an active problem of Ischemic Stroke that are not ordered a statin on the discharge med rec will trigger this pop up. • You will see the pop-up below and must either order a statin or select an override reason.
Missing Newborn Hearing Screen • For newborns born yesterday who do not have an order for a hearing test. • You will see the pop-up below and must either order the hearing screen or select an override reason.
