2. Confirmation of presence on enrollment day • Before 1.9.2014 I will confirm if I will come or not to enroll in Student Information System • I will visit this page https://is.cuni.cz/studium/eng/login.php?do=ucha • I will put my Application ID, date of birth and sex to table “Login with ID number (if you already have one)” • I will see my application and I will click on it • There will be my number of points from the test, final result “Admitted“ and details to the enrollment day • “Please fill in following form to help the university” -I will fill that table and Faculty will be able to see if I will come or not

3. When and where • 19.09.2014 • Start at 10 am (registration is at 9am) • Dean’s office, Kateřinská32, Prague 2 (Stomatology lecture hall, 2nd floor, door number 3.033) • The enrolment takes usually 4-5 hours, please don’t forget to bring something to drink and snack

4. What I should bring to the enrollment? • Confirmation about payment • Passport (ID) and the COPY of your valid passport(ID) on a paper • 3 pictures which will have PASSPORT SIZE • provide a notarized and translated copy of high school diploma (by Czech notary and to the Czech language)

6. Accommodation • We have the accommodation guaranteed for you, if you will require it • You have to let us know in advance (few months before enrollment day) and write us the exact day when do you want to come to Prague • Accommodation is in the Halls of Residence of Charles University in Prague • Usually studios are consisting of 2 double-bed rooms and common kitchen, bathroom and WC • Single rooms are not usually available • Price: around 9 EUR a day • Information and pictureshttp://kam.cuni.cz/KAM-47.html • Our students are usually in Kolej Hvezda, Kajetanka and Komenskeho

7. 8. Matriculation ceremony • 30.10.2014 • ATTENDANCE IS COMPULSORY • The official festive matriculation will be held in the big auditorium of Karolinum (Ovocný trh 3, Prague 1) • Please come in formal dress (don’t forget to pack it at home) • Teaching that day will be cancelled

9. If you won’t be able to come on enrollment • There must be some serious reason for not to come on enrollment (like illness etc.) • The maximum allowed time for starting to study on our faculty in particular academic year is 14 days • After this time it won’t possible to enrol to our faculty • Please let us know in advance if you will have any trouble to come to enroll
