  2. Go to the “New” button and click “Spreadsheet”

  3. Click the “Share” button and save spreadsheet

  4. Click the “Form” button

  5. Click the “Create a Form” link

  6. Use Data Sheet to Create an Online Form Title of Survey Instructions Question Add More Questions

  7. Example SAVE!

  8. Click “Preview and Send” and on form link to preview online reporting form

  11. EXERCISECreate your own form based on the information below! You want to collect the following school summary data from the elementary schools in your locality: • School Name • Submitter Name (Full Name of the Person Submitting this Data) • Age Group Being Reported (possible ages 10-12) • Aerobic Capacity # Tested – Boys • Aerobic Capacity # met wz – Boys • BMI Average – Boys (Average for age group) Once you have created and saved your form (use your last name to title your save), go to the form link under preview and enter in some data. Go back to the spreadsheet to see how it looks!
