  2. Incidents and Injured Employees The care of the injured employee(s) and the safety of others is your primary focus in all of the following circumstances. Paperwork and notifications are secondary, deal with the situation before the paperwork.

  3. Incidents and Injured Employees Continued • All the following information and forms can be found on the Safety & Risk Management page of the ODFnet. • Incident Manual, Volume 6, Safety • Hard copy forms and instructions are located in the administration trailer under ODF Incident Packets.

  4. Incidents and Injured Employees Continued 1.Minor Injury - No Doctor Visit • Notify Salem Safety within 24 hrs. • Fill out an Incident Analysis form (check First Aid Only). Send this form to Salem Safety and the District Safety Committee. This information provides “date of knowledge” if the minor injury becomes more severe and also provides an opportunity to learn and be proactive in correcting hazardous conditions.

  5. Incidents and Injured Employees Continued • Injury with visit to a Medical Provider (No Physician Restrictions) • Notify Salem Safety within 24 hours • Fill out SAIF 801 and an Incident Analysis form. Fax SAIF 801 to SAIF within 5 calendar days. Send copies of both SAIF 801 and IA to Salem Safety and a copy of IA form only to the District Safety Committee.

  6. Incidents and Injured Employees Continued • Employee is Released to Return to Work (On Modified Duty) • Notify Salem Safety within 24 hours • Fill out SAIF 801 and an Incident Analysis form. Fax SAIF 801 to SAIF within 5 calendar days. Send copies of both SAIF 801 and IA to Salem Safety and a copy of IA form only to the District Safety Committee. • Complete Job Description Form and Optional Job Offer Form. Send information to Salem Safety within 5 days.

  7. Incidents and Injured Employees Continued 4. Employee is Not Released to Return to Work • Notify Salem Safety within 24 hours • Fill out SAIF 801 and an Incident Analysis form. Fax SAIF 801 to SAIF within 5 calendar days. Send copies of both SAIF 801 and IA to Salem Safety and a copy of IA form only to the District Safety Committee. • Analyze completed Release to Return to Work form and ensure it signifies employee is not released for modified duty potential. • Submit proposal to Salem Safety Manager using a Job Description Form for modified duty to medical provider for authorization. • If accepted supervisor will prepare appropriate Mandatory Job Offer or Optional Job Offer form. Deliver or send Certified Mail to the employee. • If declined, forward copies of the job offer form to the Salem Safety Manager and SAIF.

  8. Incidents and Injured Employees Continued 5. Injury with Overnight Hospitalization • Notify your supervisor and Salem Safety Manager as soon as possible. They will contact SAIF, OR-OSHA and Public Affairs. • Fill out SAIF 801 and an Incident Analysis form. Fax SAIF 801 to SAIF within 5 calendar days. Send copies of both SAIF 801 and IA to Salem Safety and a copy of IA form only to the District Safety Committee.

Incidents and Injured Employees Continued • Illness - Vehicle Accident - Catastrophic Injury (2 or more fatalities, 3 or more hospitalizations) • Notify your supervisor and Salem Safety Manager as soon as possible. Notifications are required within 8 hours to SAIF, OR-OSHA and Public Affairs. The Safety Manager will make these notifications, but you will need to provide the following information: • Date, time and location of incident • Employer name, address and phone • Description of incident and injuries • Name, occupation and date of birth, as well as contact information for all injured parties

  10. Incidents and Injured Employees Continued 7. Fatality • Notify law enforcement immediately • Do not move the individual unless there is danger of evidence being destroyed. • Do not release victim name, except to authorities. • Do not allow unauthorized pictures. Only take pictures if the victim must be moved. • Notify your supervisor and Salem Safety Manager as soon as possible. Notifications are required within 8 hours to SAIF, OR-OSHA and Public Affairs. The Safety Manager will make these notifications, but you will need to provide the following information: • Date, time and location of incident • Employer name, address and phone • Description of incident • Name, occupation and date of birth. Contact information for all injured parties
