City of LaGrange Health Care Plan • Self-Insured • Type of Insurance Arrangement where Employers Pay for Medical Claims out of their own Funds rather than Contracting with an Insurance Company for Coverage. This puts the Employer at risk for its Employees' Medical Expenses rather than an Insurance Company. • TPA (Third Party Administrator) • An organization that processes health plan claims but does not carry any insurance risk (Covenant Administrators)

City of LaGrange Health Care Plan • Re-Insurance- One insurance company (UHC) agrees to indemnify another insurance company (the City of LaGrange) in whole or in part against risks that the first company has assumed. • Specific Stop Loss/Reinsurance - insurance which pays for medical charges above a selected deductible for an individual person per policy year. $175,000.00 • Aggregate Stop Loss/Reinsurance - provides coverage against an entire population's budget overrun in a calendar year. $2.6 million

  4. City of LaGrange Health Care Plan • PPO (Preferred Provider Organization) - • A doctor, hospital or other healthcare provider who has entered into an agreement with United Healthcare to provide healthcare services to members for a negotiated rate of reimbursement.

  5. Basic Plan Design • Family or Single Coverage • $20.00 Office Visits – In Network • Deductibles • $250 - Single • $500 – Family • Co-Insurance • 80% after Deductible up to $5,000 of Services – In Network • 60% after Deductible up to $5,000 of Services – Out of Network

Goal for Committee • Organize 6 projects to promote wellness within the organization • First Project – 8 in 8 • 8 Week Program to encourage employees to lose 8 pounds in 8 weeks • 84 of 105 participants completed the program • 1,190.5 total pounds lost • 14.17 pounds per person lost • 11 of 34 Teams lost 40 or more pounds • 84 of 380 (full-time insured) employees completed the program – 22%

Second Project – 500 Mile Club • 500 Mile Club • Program designed to Encourage Physical Activitity • Walk, Run, Elliptical, and/or Bike (4 miles equals 1 mile of walking) your way to 500 Miles! • Initial Registration Dates – March 31 – April 18 (We will have more registration dates later in 2008 that will give people another opportunity to register for the ones who did not March 31 through April 18) • 1 Year to complete the program (Program begins Monday, April 19, 2008 and ends Monday, April 20, 2009) • Mileage Log Sheet (Given to each participant once he/she registers to keep up with mileage)

  23. Lunch and Learn The Health Committee will hosting several lunch and learn programs during the upcoming holidays. These programs will cover some very important issues. Building Alcohol (LaGrange PD) December 12 11:00 Stress, Holiday Diet (Dr. Smalls) Municipal Court

  24. Join the fun on Saturday, December 13 at 8:30am for a Poker Run! Starting on the Square, run/walk a 3.1 mile course, as you meet a dealer with playing cards along the way. To complete your hand, the final dealer and cards will be waiting for you at a nearby station. This is one group run/walk, where you can win with your hands rather than your feet, as the best poker hand, not the fastest runner, will win an iPod Shuffle! Other prizes available too. After the run, enjoy the award ceremonies and door prizes Registration Fee: Donate new toy for TOYS FOR TOTS. So, ask yourself, do you feel lucky?

  25. Chief’s 10K Challenge STEP UP TO THE CHALLENGE! Join Chief Lou Dekmar for his 2nd Annual Chief’s Running Challenge! Saturday, December 22, 2008 Fire Station #4 Mooty Bridge Road 10:00 a.m. All participants who register prior to Monday November 24th will receive an event t-shirt. Lunch to be provided at Station 4 at the conclusion of the event.
