        • CTX/TB status
        • Patient status i.e. Lost, dead, etc
  • ART Monthly Register
      • Unique number has MFL prefix
      • Extra columns for pregnancy (ies) while on ART
      • Columns (ab) to (bk) from months 25th-48th month. The upper part of the cell is for month 1st-24th while the lower part is for 25th-48th month

      • Replaced with the coded facility reporting form with data elements HV03-01-HV03-55
  • Summary Sheet -ART Cohort
      • Added rows on:
        • Fraction CD4 < 100 (of adults with available CD4 at baseline)
        • Fraction with CD4% < 15% (of children <5 with available CD4 at baseline)
      • Dropped row on:
        • Number of persons who picked up ARVs each month for 6 months
  • Activity Sheet & Tally Sheet are new tools
