  2. Purpose of the Program: • Student and/or stakeholder needs addressed by the program • Staff prepared to provide attention to students • School and family partnership focused on healthy students • Compliance with all laws related to student health

  3. Service Delivery Model • Qualified staff provide appraisals and screenings . • Job related knowledge and skills annually updated • Electronic data base provides consistent and readily available health related information • School and family work collaboratively when specialized health plans are needed.

  4. Program Deployment Status • Services are fully deployed without significant weaknesses or gaps in any school or area

Progress Toward Previous Program Goal GOAL: The Park Hill Health Services program will achieve 100% compliance with state guidelines related to student vision and hearing screenings (K, 1, 3, 5) RESULTS: As of December 15, 2011: 86% of students had received Vision Screening; 89% had received Hearing Screening. Did not meet goal of 100%.

  6. Progress Toward Previous Program Goal GOAL: A survey will be developed for students and parents to gather feedback on the services provided by the Health Services Department. RESULTS: A survey was developed and deployed to gather parent feedback related to the services provided by the Health Services Department. The student survey was delayed for deployment until 2012-2013 as students were participating in the Missouri Safe and Drug Free Survey, which is deployed on a bi-annual basis. Goal met to develop Parent Survey. Student survey will be deployed in 2012-2013

Progress Toward Previous Program Goal GOAL: Health Services staff will receive professional development training in current student health issues (i.e. diabetes, allergy prevention, current student drug use trends and use of Show-Me Vax program. RESULTS: : Training was provided in all identified areas. Goal was met. All staff members participated and received follow-up materials.

Future Program Goals to be Evaluated • [Goal 1] The Park Hill Student Health Services program will increase compliance with state guidelines related to student vision and hearing screenings (Grades K,1,3, and 5) by December 15, 2012 to the following levels: Hearing 92%; Vision 90%. 70% of 7th and 97% of 9th grade students will receive a vision screening (new goal). • [Goal 2] Parent Survey will report an increase in the level of ‘Health Services staff interacts with parents in an inviting and helpful manner’. • [Goal 3] Student survey will be deployed to determine baseline levels of program satisfaction.

Plan for Evaluation Measurable Outcomes Data Collection Methods • Outcome 1: SNAP Data will be collected to verify a screening increase of hearing to 92% and vision to 90% for students in grades K, 1, 3, and 5) by December 15, 2012. Middle School vision screenings will increase to 70% and High School vision screenings will increase to 97%. • Outcome 2: The Parent Health Services survey question ‘The Health Services staff interacts with parents in an inviting and helpful manner’ will increase from 77% to 85%. • Outcome 3: A student survey will be deployed to gather feedback to establish baseline levels of program satisfaction. • Document review • Data review • Surveys • Interviews • Department meetings
