Building Community Relationships • Develop talking points for your organization’s executive team, leadership team, and service team • Develop promotional materials • Host a kick-off event to inform and engage community partners • Encourage collaborative problem-solving with stakeholders • Invite community stakeholders to call the ACT Team for proactive responses to clients

Referral System Assessment and Monitoring (RSAM) • Toolkit originally designed for use among HIV/AIDS service providers to improve referral systems • Now being used for other community health networks • The RSAM toolkit assists in deciding • Which tools to use • How to adapt the tools • How to use the tools for data collection • How to analyze, interpret, and use the information generated

Component 2: Referral System Management “RSM” • Monitoring system to track the performance of a referral system over time • Whether or not referrals are being made and completed • Whether counter-referring is occurring • Level of client satisfaction with the referral process • Allows for identification of problems and adjustment
