Component 1: Referral System Assessment “RSA” • Tools & Instructions for … • Assesses current status of the referral system • Evaluating the processes used to document referrals • Identifying areas that require improvement • Focuses on systems and processes for effectiveness of the referral system • Can be modified to examine appropriateness of referrals • Focuses on functioning at a single point in time
Component 2: Referral System Management “RSM” • Monitoring system to track the performance of a referral system over time • Whether or not referrals are being made and completed • Whether counter-referring is occurring • Level of client satisfaction with the referral process • Allows for identification of problems and adjustment
