All visiting consultants from NUH. Runs CTVS CL on Wed and Fri Only seeing reconstructive surgery Aesthetic procedures done at Clinic 2B Cardiothoracic (Heart)

  3. How to make appointments • Patients must be referred to the clinic by: • Polyclinics • Specialist Outpatient Clinics (SOCs) • TTSH Wards/ Renci Hosp/ Rehab wards • Other restructured hospitals (RH) • GPs under the CHAS scheme

Appt Urgency for Polyclinic Referrals • Routine • Mostly handled by Contact Center staff unless no appointment slots can be found (usually for Plastic / CTVS cases) • Direct Access (NHG) & Fast Track (SingHealth) • Appointments that are deemed more urgent • Doctor to screen for TCU duration • Appointment to be given within TCU duration stated No further action required after giving appointment in system UNLESS appointment given is in short notice Indicate in comment for DA / FT appointments: DA (name of polyclinic) (date of referral) SCR (doctor’s name) TCU (duration) eg, DA HOUGANG POLY SCR DR JULIAN TCU 2/52

  11. TTSH Emergency Dept referral • 3 types of referrals for all disciplines (CVM, CTVS, Plastic) • Routine / Early / Urgent – HOT • Routine ED Referrals handled by ED staff • Early or Urgent – HOT referrals to be screened by Dr before making appt • Urgent – HOT appt to be faxed back to ED and recorded in file *Check for existing appt or open case before sending it for screening*

  12. Calling wards within TTSH Alternatively, the ward contact number can also be found at the top right hand corner of the discharge appointment
