Procedural Requirements • Applied Materials procedure 0250-36352 is the procedure used to define control requirements for critical dimensions. • Procedure Title: Certificate of Compliance and Final Inspection Report Requirements. • The Certificate of Conformance and Final Inspection Report procedure states that when critical dimensions need to be controlled, drawings must reflect the following drawing note on the drawing: • CERTIFICATE OF COMPLIANCE AND FINAL INSPECTION REPORT REQUIREMENTS PER APPLIED MATERIALS 0250-36352. • Example of Critical Part identification for C of C requirements as shown on drawing or engineering specification • Example of drawing note requirements for C of Cs and FIRs • Final Inspection Reports or FIR as they are commonly referred to will appear as a separate page within the Applied Materials drawing package. • 0250-36352 defines an FIR as follows: • The FIR is part of the drawing/specification and defines the critical attributes measurements required by the supplier to record. The FIR drawing sheet (which is a CAD drawing) must be used. Applied Materials Confidential

Control of Critical Dimensions and FIR • General serialized parts are to comply with the requirements stated in 0250-36352 • Part and assemblies requiring serialization must be inspected 100% and include with their shipment an individual Certificate of Compliance. • Record retention for serialized parts must be for each serial number • Non-serialized FIR part requirements are to comply with the requirements stated in 0250-36352 • When non-serialized parts are manufactured as a controlled lot, a sampling plan using a minimum of 1.0 Single Sampling Plan, level II (ANSI/ASQC Z1.4 or equivalent) may be used. The lot must be traceable by work order number or lot number. One C of C for the lot shall be completed and a FIR(s) completed for each of those parts selected per the AQL. • If parts are manufactured in a non-lot controlled process, each part must be 100% inspected to the specified critical features required by the FIR and an individual Certificate of Compliance completed. • Record retention for non-serialized parts must be for each lot. • Record Retention are to comply with the requirements stated in 0250-36352 • The Final Inspection Report or Test Documentation (as required for OEM parts by the Supplier Quality Engineer) and a signed copy of the corresponding Certificate of Compliance must be maintained by the supplier for a period of three years. The supplier must furnish Applied Materials, within 24 hours of a request, a copy of the Final Inspection Report or Test Documentation. • Additionally, an complete FIR must be included with each FAI submitted for review when required. The FAI must remain on file with the supplier for 6 years which is in accordance with 2-60.1. Note: Final Inspection Reports must not be shipped with Spare Part shipments. Certificates of Conformance (C of C) • If the C of C and Final Inspection Report procedure is noted on the drawing, the following requirements must be followed to comply with C of C requirements as stated in 0250-36352. • 0250-36352 defines a certificate of conformance as a guarantee in writing that the purchased item has been produced in accordance with Applied Materials requirements. • The supplier must include a signed Certificate of Compliance with each part. The C of C shall be placed inside the packaging box. The outermost packaging must be labeled adjacent to the part number with the following: “Certificate of Compliance Enclosed”. • The C of C can be miniaturized into a label, but must be legible. • When submitting the C of C this document takes precedence over all other Certificate of Compliance related Specification documents. • Lot or Batch produced - critical parts, All non-serialized critical parts, produced in lots or batches, must have a corresponding copy of the original C of C included with each part. Non-serialized critical parts, produced in lots or batches, received without C of C in the correct quantity will be rejected at incoming inspection and returned at the supplier's expense.
