Security • Locked doors still a problem; EBIS, Booster doors often left unlocked; Building 912 doors often chocked open • Contractor Training Room location may lead to a problem; contractors are allowed on-site at 7:30 but class starts at 8:30 • Contractors wander C-AD areas at that hour since the contractor-training building is in the remote regions of C-AD

  3. Personnel Protective Equipment (PPE) in Machine Shops • Machine Shops at C-AD will be placed under boundary PPE rules • Conditional PPE no longer allowed in machine shops by BNL; some exceptions • Safety glasses, safety shoes to enter • Peter C. will evaluate • Temporary posting in the next week

Yale Accident in Machine Shop • A Yale University student from Massachusetts died in an accident last Tuesday night at the Sterling Chemistry Laboratory • Michele Dufault’s hair got caught in a lathe, a piece of machinery that spins very quickly, and it pulled her in, sources said • The state medical examiner’s office reported that Dufault died from accidental asphyxia by neck compression • No causes determined yet; OSHA investigating • Michele was working alone late at night (2:30 am); working to finish senior project

Action Items Proposed by BNL • Review policies and procedures on long hair, lanyards and loose clothing when near rotating equipment • Ensure proper guards protect workers from rotating equipment • Review training requirements for use of shop tools; especially for students and new workers • Assurance barriers at C-AD: • Training and qualification program, warning signs, disciplinary policy, work planning, Manager Work Observations, pre-job briefings, machine guards, PPE, hazard assessments, housekeeping, safety inspections • Missing barrier – zero machine shop safety requirements in SBMS • One web based training in BTMS on hand and power tool safety; not much on rotating equipment safety or machine shop safety • No information in C-AD OPM, C-AD Access Training or Users Training on machine shop safety; all refer to SBMS Subject Area on Machine Shop Safety
