  3. Patient centered medical home • Medical Staff: Medical Director Paul Schaefer MD, PhD and Mariam Mina MD • Unique care delivery model. Patient care coordinated and centralized. Facilitates partnerships. Care that values preventative, physical and mental health. Treatment of acute and chronic health needs. • Offers shorter wait time for urgent needs • Alternative communication methods • Laboratory services and pharmacy available on site • Hours of service: M-F 8:30 am – 5:00pm • 419-530-3580 to speak directly to staff and make appointments

Main Campus Medical Center • Medical Director Sanford Kimmel, MD • Service to students registered at least one semester and faculty/staff with acute illness or injuries • Services include primary care, gynecology, mental health, vaccination, immunization and allergy injection clinic by appointment • Hours of service: • Student clinic Mon – Thur 8:15 am to 8:00pm Fri 9:00am to 5:00pm • Summer and breaks periods 1-5 pm

Family Medicine at the RupPert Health Center • Staffed by Board-certified Family Medicine, faculty Lead by Dr. Linda Speer • Services for newborn to elderly • Routine care of any illness, gynecology, acute careand chronic illness, health maintenance • Employee health services: pre-employment requirements, respiratory fit testing, • occupational exposure • Hours of service: Mon – Fri 7:45 am to 5:00 pm

Student health and Wellness Center at The RupPert Health Center • Staffed by Board-certified Family Medicine Faculty, nurse practitioners, and mental health providers • Service to all students registered for at least one semester hour • Fulfill primary care as well as acute needs, physical exams, vaccinations, screenings for program requirements, mental health services • Appointment hoursl: • Mon and Tue 12:00pm to 6:00pm • Wed and Thur 12:00pm to 5:00pm • Friday 9:00am to 1:00pm

Talmadge road Family Physicians Primary care • Lead by Dr. Swapna Palla • Located at the corner of Talmadge Rd. and Sylvania Ave. • Care of newborns to elderly • Laboratory, hours of operation: Mon – Fri 7:00 am to 7:00 pm • Routine care of acute and chronic illness, health maintenance • Special interest in women’s health and preventative medicine • Hours of service: Mon through Thur 8:00 am to 5:00 pm; Fri 7:00am to 4:00pm

Fallen timbers family physicians primary care • Lead by Dr. Chayanika Pal • Located at The Fallen Timbers Mall • Care of newborn to elderly • Routine care of acute and chronic illnesses, health maintenance • Special interest in women’s health and preventative medicine • Laboratory hours : Mon – Fri 7:30 am to 7:30 pm, Sat 10:00 am to 2:00 pm • Hours of operation: • Daytime: Mon, Tues, Wed, Fri 8:30 am to 5:00pm; Thur 7:30 am to 3:30 pm • Evening: Tuesday and Thursday 5:00 pm to 7:00 pm
