  2. Root cause analysis:- Root cause analysis (RCA) is a process for researching “root causes” of problems and a method find. RCA is based on the technique that requires more than just finding problems that develop, but finding a way to avoid.

  3. Goals :- • The main goal of using RCA is to analyze problems to identify: • What happened • How it happened • Why it happened • Responses for preventing to reoccur can be developed in a possible manner

  4. Benefits • Identify causes of problems, so that remedy can be developed. • Identify current state and future requirements for organizational improvement. • Establish repeatable, processes, in which one process can confirm the results of another process.

  5. Basic method • Derive the problem. • Collect information. • Identify relevant issues that caused the problem. • Identify root causes. • Identify recommendations for eliminating or minimizing the reoccurrence of problems. • Implement the found solutions.

  6. RCA methods • Few methods used in RCA are: • The “5-Whys” — A simple problem-solving method that helps users get to the root of the problem in a very quick time. This method involves looking at a problem and asking “why” and “what caused this problem”. Often the answer to the first “why” triggers a second “why” and so on providing the basis for the “5-why” analysis.

Barrier Analysis — Findings that involves tracing of route by which a target is affected, including the knowing of any missing countermeasures that could or should have prevented the undesired outcomes. • Change Analysis — Looks possible risk impacts and designated risk management strategies in situations where change is there. This also includes system configurations are changed, operating practices or policies are changed, different activities performed, etc

  8. Fish-Bone Diagramderived from the quality management process, it’s an analysis tool that provides a proper way of looking at effects and the causes that create problems. The design of the diagram is similar to a  skeleton of a fish so  named it as  “fishbone” diagram.
