Open Enrollment • Open Enrollment (OE) • One time a year opportunity • All Benefit-eligible employees can make changes to: • Medical • Dental • Vision • Accidental Death & Dismemberment Insurance • Added dependents require proof documentation for enrollment • Changes made during OE will be effective July 1, 2011

  4. Special Enrollment for Dependents Up to Age 26 • Special Enrollment • 30 day enrollment mandated by Healthcare Reform • Benefit-eligible employees may add adult dependent children for the following reasons: • Coverage ended • Coverage was denied or dependent was not eligible • Coverage of children ended before attainment of age 26 • Applies to Medical, Dental, and Vision • Proof documentation is required for enrollment • Changes made during Special Enrollment will be effective July 1, 2011

Enrollment Periods • Open Enrollment • Wednesday, April 27 – Wednesday, May 11 • Online enrollment only via LoboWeb • Special Enrollment for Dependents Up to Age 26 • April 27 – May 27(Overlaps with OE) • Online enrollment only via LoboWeb - April 27 – May 11 • Paper enrollment form only - May 12 – May 27

Proof Documentation Requirement • Required if adding a dependent • Examples available on Open Enrollment Web page • Required documents must be submitted to HR Service Center by 5:00 pm on Thursday, June 30, 2011 • Failure to submit documentation by designated time will void dependent enrollment

  7. Enrollment Communications & Resources • Email notices from the Division of Human Resources • Mailing to home addresses of all benefit-eligible employees • Dedicated Open Enrollment Web page providing enrollment information, links to details, OE sessions, plan design changes, and rates • Service Center Support: 277-MYHR(6947) and 277-HRPR(4777)

  8. Information Sessions • Open Enrollment information session times and locations will be posted on Human Resources Open Enrollment Web site • Benefits office is available for department presentations • Minimum attendance of 10 employees
