  2. はじめに - Introduction この度の東日本大震災は、マグニチュード9.0で、 しかも全長約500kmにわたる震源域というかつてない規模の 大地震であった。 The East Japan Great Earthquake with a 9.0 magnitude was unique in terms of its epicenter which spread out over 500 kilometers (310 miles). 震源のひとつであり、福島第一原発から45kmに位置する福島県いわき市の病院で、震災後どう対処したかを振り返る。 One of the epicenters was off shore from Iwaki City, located only 45 km (28 miles) from Fukushima Daiichi Nuclear Power Plants. In this presentation, we will examine how one hospital functioned after the disasters. それにより、われわれ医療従事者はどう行動するべきであるのか、阪神・淡路大震災と比較して考察した。 By doing so, we will be able to compare with the KobeEarthquake, and discuss how medical provider should deliver services during disaster.

  4. 震災翌朝- Next Morning 東京に戻れず数日間現場で医療活動を行った非常勤医師達。 Part time doctors, who could not return to Tokyo, worked with us for several days. 携帯電話やメールがまったくつながらなかった。I-padは非常に有用であった。 Cell phones, email, and texting did not work at all, but iPads were very useful. 倒れた病院外壁 Fallen wall of the hospital

  5. Choushunkan Hospital 海岸沿いの関連病院で、1階が津波に流された。 患者は全員無事であったが、職員1人が津波で死亡した。Affiliated hospital located by the coast. 1st floor completely engulfed by Tsunami. Patients survived but one worker killed by Tsunami. 3か月後の写真だが、車などそのまま放置状態で あった。Three months later, damaged cars are still abandoned.

  6. Usuiso Beach in Iwaki 3か月後の写真。Usuiso beachは小さな集落だが、津波がすべてを飲み込んだ。 津波警報が鳴らなかったため、多くの住民が逃げ遅れて死亡した。 Three months after the disaster. Usuiso is a small village and the Tsunami swept everything away. The Tsunami warning did not go off and many people could not escape and were killed.

Matsumura General Hospitalでの震災当日 What happened on the day of the disaster at Matsumura General Hospital 松村総合病院は8階建ての病院である。 Matsumura General Hospital has 8 floors. 非常勤医師5名は東京に戻れないため、そのまま数日間医療活動を行った。 5 part-time doctors could not return to Tokyo. For several days, they stayed and took care of the patients. 私を含め2名の医師は、出張先からタクシー、ヒッチハイクで当日および翌日に病院に戻れた。 2 full time doctors (including myself) came back from business trip destinations by taxi and hitchhiking on that day or next day. 東北電力エリアのため、停電はなかったが全エレベーターが停止した。 There was NO power outage since the region was serviced by Tohoku Electric Co., but all hospital elevators stopped. 病院倒壊の恐れがあったため、まず入院患者を担架と毛布で院外へ運んだ。 Since the hospital building was in danger of collapsing, all patients needed to be brought outside by stretchers and blankets.

Matsumura General Hospitalでの震災当日What happened on the day of the disaster at Matsumura General Hospital 挿管中の患者はいなかったが、重症敗血症の患者1名は外に出せなかった。 There were no patients with breathing tubes, but one patient with sepsis (severe infection) needed to be kept inside the hospital. 安全確認の後、一旦患者をもどした。 After confirming safety of hospital building, all patients brought inside. その後、救急隊より溺水患者の搬送があり、1階フロアを開放し対処した。 As many people who nearly drowned were brought here by rescue teams, we opened the 1st floor and created space for the new patients. 津波による患者20人中4人は軽症であった。入院患者3名のうち2名は救命できた。残りは救命できなかった。 Among 20 Tsunami victims, 4 were mild cases. Among 3 patients who were hospitalized, we saved 2, but we could not save 1. 呼名反応あるものの低体温により救命できなかった患者もいた。 Some patients, who were able to respond to their names, could not be saved because of hypothermia.

The Patient with Tsunami-Lung 51歳警察官。 51-year old police man. 3月11日救助作業中に津波にのまれ溺水。 During the rescue effort on 3.11, he was engulfed by the Tsunami and nearly drowned. 海沿いの病院で入院し、点滴酸素療法を受けるも状態悪化し、3月12日当院転送となった。 Hospitalized near the coast and received oxygen therapy. Transferred to our hospital due to worsening symptoms. 胸部X線、CTにて肺炎、肺水腫であった。 X-ray detected lung inflammation /pulmonary edema

The Patient with Tsunami-Lung 入院時所見:Hospital Data 意識清明、体温37.9℃、血圧117/66mmHg、 Alert, temp = 37.9C, BP = 117 / 66 脈拍78回/分、全身に外傷あり。 Pulse = 78, injuries over entire body sPO2 91%であったが、酸素投与で97%まで上昇。 SpO2 = 91%, up to 97% with oxygen therapy 全身疼痛、胸痛、呼吸苦あり。 四肢冷感著明、爪色不良。 General body pain, chest pain, and respiratory distress. Coldness of limbs and nail discoloration. 3月13日血液検査 Blood test on 3.13.11にてLDH 268 (176-353)、TP 6.3、CRP 20.2 (elevated) であった。

The Patient with Tsunami-Lung 入院後、エンペリックにクリンダマイシンと メロペネムを投与し、症状は軽快傾向 であったが、抗生剤がなくなったため、転院となった。 After admission for empiric therapy, Clindamycin and Meropenem were started and symptoms became less severe. Then, we ran out of antibiotics so patient was transferred to another hospital. その後約2ヶ月間の入院加療の後、退院 となった。 After two months of inpatient care, he was discharged.

  12. 震災後の状況-Situation after the Disaster ライフライン:停電はなかったが、水道がでないため、貯蔵タンクの水の使用のほか、浄水場までいき配給を受けた。トイレは、簡易式トイレを作り対処した。食料は、災害用のパンの缶詰で対処した。風呂、シャワーは2週間近く使用できなかった。 Life-Line: NO power outage, but NO running water. We used water from storage tank and from water purifying plant. We created temporary toilets, and obtained food from disaster supply. No bath nor shower for two weeks. スタッフ:医師は、出張途中に震災にあうもタクシーで7時間後病院にもどったものや、避難所からヒッチハイクで翌日もどったものがいた。その反面、2日後に家族と遠方へ1ヶ月以上避難した医者もいた。残った医師は、全員病院内に1週間以上宿泊した。 Staff: One doctor came back from business trip destination by 7 hour taxi ride and other doctor by hitchhiking from rescue center. All doctors remained in hospital over 1 week. Additionally, one doctor who escaped 2 days after the disaster to a distant location returned after one month. コメディカル:多くの看護師は家族とともに避難し、約半数の看護師が働けなくなった。ガソリンがないため、震災から最長2週間病院に泊り込みで勤務した看護師もいた。人不足のため3月いっぱいは通常の3交代制から2交代制にした。 Other healthcare professionals: Since many nurses were evacuated with their families, only half of the nursing staff was available. Due to lack of gasoline, some stayed in hospital for as long as 2 weeks. They worked on a two-shift system during March 2011.

  13. 震災後の状況-Situation after the Disaster いわきは地震に加えて原発が近いことからか、1週間全く援助がこなかった。行政が現状の危機をメディアに報告していなかったため、独自にyou tubeに情報を流し、メディアのインタビューで危機を報告し、ようやく自衛隊からの水、食料がとどいた。 In addition to earthquake and Tsunami, since Iwaki City located very close to nuclear power plants, no rescue effort provided for two weeks. Due to a lack of information released to media by government, we broadcasted very urgent messages via YouTube and were interviewed by media. After all these efforts, Defense Army finally delivered food and water to us. 震災後は、海沿いの津波被害にあった関連施設からの患者の受け入れや、原発非難区域にある他院からの重症患者の受け入れも行った。しかし、1週後には薬剤が枯渇したため、重症患者は転送した。転送手段は、ガソリンがなく救急車が稼動できなかったため、家族の自家用車や、ヘリコプターで行った。 After 3.11, we received many patients from associated facilities, some near the coast and others located in evacuation areas near the damaged nuclear power plants. After a week, major medications were all used up and did not get replenished. Because of that, many patients with critical conditions had to be transferred to hospitals in other areas. Due to the scarcity of gasoline, ambulance vehicles were not in operation. So, we ended up using private automobiles and helicopters.

  16. 大震災後におこりえる経時的病態 Time line of The Clinical State Following Major Disaster • トリアージを要する救急患者 (地震発生~48時間) • Emergency Cases require Triage (Time of Earthquake ~ 48 hours) • 2. 脱水・血栓症・感染症 ・低栄養 (48時間~2週間) • Dehydration・Thrombotic Disease・Infectious Disease・Malnutrition (48 Hours ~ 2 Weeks) • 3. 心的外傷後ストレス障害 (Posttraumatic stress • disorder:PTSD) (2週間以降) • PTSD(After 2 Weeks)

  19. 結果 - Result 今回の震災は、津波による溺死がほとんどであり、 トリアージの必要性がないくらい生死がはっきりしていた。 In this disaster, most death caused by drowning. In many cases, triage not needed since there is a distinct division between the dead and those alive. 救命できた溺水患者も、津波肺という特殊な肺炎状態であり、回復が遷延した。 Patients saved from drowning, later developed “Tsunami Lung” and suffered prolonged recovery course. 原発問題がからみ、ライフラインを含めて援助対策が不十分であった。 With complexity due to the Nuclear Power Plants accidents, we must say the recovery and reconstruction policy was not adequate. Especially for re-building the lifeline. ガソリン不足のため、後方支援病院への転送手段がヘリコプターであった。 Due to the lack of gasoline, only transportation for patients to other hospitals in safer areas was helicopters.

  20. おわりに - Conclusion 大震災における人間の行動は一様であり、自らの生命を 守る責任は自分にあると考えれば、いずれの行動も責めるわけにはいかないと感じる。 Human behavior in the great disaster may not differ from each other. By making the point that we all have responsibility to protect our own lives, we cannot blame anyone concerning any behavior they took to save their own lives. しかし、今回の震災において、自己犠牲精神のため命を 落とした方が多数いる。 On the other hand, in this disaster, many individuals lost their lives in the sprit of self-sacrifice. 日本人は、古来より仁義、情、武士道精神を有していると 考える。 I think that Japanese people continue to carry the value on humanity, compassion and Bushido (spirit of samurai).

  21. おわりに - Conclusion 多くの人間を統括する立場にいる人は、あえて自らを かえりみず、現場へいき、滅私奉公の精神で仲間を守ろうと するであろう。 In an emergency situation, person who is in charge of many people may rarely think twice about him or her. Instead, they will sacrifice themselves for the sake of securing the safety of others. その気持ちが、復興の活力となり、平穏に戻った時の信頼関係になると信じている。 I believe that this spirit will lead us to generate social vitality, and form the solid foundation for the relations of trust when we gain back peacefulness.
