  2. Host Home Program NeuroRestorative’s innovative Host Home Program provides participants with the opportunity to transition from facility-based services to life in the community. Participants will live in the home of a primary support person known as a Mentor. We carefully match participants with Mentors and strive to create meaningful relationships that lead to lasting success for program participants and the Mentors they live with.

Service Provision • Continuous evaluation of discharge plan • Community Integration • Family Training & Education • Social & Recreational Activities • Vocational Rehabilitation • Medication, medical and disability management • Overall coordination of community resources and supports Intended outcome: evaluation of independent living.

  5. How is Host Home different? • Intensive oversight by RN and Case Management • Recruitment of Mentors • Pay for Caregivers • Person-centered training/admission • Initial referral to Mentor • Match meeting • Pre-admission Meeting • Placement • Short-term transitional services
