Our comprehensive clinical team provides oversight and support tailored to each participant's needs, including life skills training, vocational rehabilitation, and community integration. We focus on empowering participants towards independent living while continuously evaluating their progress in a nurturing environment.

Host Home Program NeuroRestorative’s innovative Host Home Program provides participants with the opportunity to transition from facility-based services to life in the community. Participants will live in the home of a primary support person known as a Mentor. We carefully match participants with Mentors and strive to create meaningful relationships that lead to lasting success for program participants and the Mentors they live with.

Service Provision • Continuous evaluation of discharge plan • Community Integration • Family Training & Education • Social & Recreational Activities • Vocational Rehabilitation • Medication, medical and disability management • Overall coordination of community resources and supports Intended outcome: evaluation of independent living.
