Off-Pump vs. On-Pump CABG: Short-Term and Long-Term Outcomes

This study compares the composite outcomes of off-pump vs. on-pump CABG procedures, showing no significant difference in short-term outcomes but worse long-term results with off-pump, including higher cardiac death rates and lower graft patency at 1 year. The trial design involved patients scheduled for elective or urgent CABG, randomized to off-pump (n=1,104) vs. on-pump (n=1,099) surgical revascularization, with a 1-year follow-up. Findings suggest on-pump procedures may be more beneficial in the long run.

ROOBY Short-term composite outcomes: 7.0% with off-pump vs. 5.6% with on-pump (p = 0.19) Long-term composite outcomes: 9.9% vs. 7.4% Cardiac death: 2.7% vs. 1.3% Graft patency: 82.6% vs. 87.8% (p < 0.001) Trial design: Patients scheduled for elective or urgent CABG were randomized to off-pump (n = 1,104) vs. on-pump (n = 1,099) surgical revascularization. Follow-up was 1 year.
