DREAMING in a nutshell • DREAMING is one of the very first Pilot B projects funded under the new European Union CIP-PSP (Competitiveness and Innovation Programme – Policy Support Programme)‏ • Project start date: 1st of May 2008 • Project duration: 36 months • Total budget: 5.540.000 Euros • EU contribution: 2.770.000 Euros • 6 real life trials in Denmark, Estonia, Germany, Italy, Spain and Sweden 4

OBJECTIVE • EVALUATE a new health service in order to OFFER comprehensive assistance to chronic patients at home, WITH THE HELP of new information technology and communications. • Comprehensive care: OMS criteria. Meeting the physical, psychological and social aspects of patients. • USE CASE: elderly fragile +65 • Definition: • Medical criteria: pluripathology, poly-pharmacy, recent hospitalization (admission in the last year), readmissions (2 or > in the last year), falls (1 or more in the last 6 months), diseases as dementia, neoplasia, stroke, heart failure, depression • Functional criteria: elderly with none independency for doing daily life activities • Socio-demographical: aged more than 80 with difficulties for receiving help (living alone)‏ 8

INITIAL EVALUATION Differential features with regards to other pilot sites All patients (test and control group): • (SF-36) to measure patient health and well-being. • (HADS) to measure anxiety and hospital depression. • Philadelphia indicator to measure lifestyle satisfaction. • Pfeiffer questionnaire in order to measure the degree of cognitive impairment. • Lawton-Brody scale and Barthel index to measure the degree of dependence. 13
