Project Sentinel Collaboratory Georgetown University Medical Center Washington Hospital Center

This project aims to develop a systematic approach to provide timely indication and warning of impending infectious disease threats across plants, animals, and human populations. It will leverage data from various sources, including environmental and climatic factors, to enable rapid prevention and response. The project is funded by the National Library of Medicine and led by Dr. Seong K. Mun at Georgetown University Medical Center and Washington Hospital Center.

  1. Project Sentinel CollaboratoryGeorgetown University Medical CenterWashington Hospital Center Seong K. Mun, PhD Funded By National Library of Medicine

  2. ISIS Approach to Integrated Biodefense • We need a systematic way to provide • Indication and Warning (I&W) of impending threats from • infectious diseases across our • plants, • Animals and • human populations • So prevention can be rapidly undertaken

  3. WNV: Relatively Low Level Threat Excellent Model for Enviro-climatic Coupling of Pathogens Surrogate Model for Propagation of Mosquito Mediated Infectious Disease

  4. Source: CDC 3

  5. I&W : Venezuelan Equine Encephalitis (1995) “Conditions Favorable”: enviro-climatic signature tracks with epidemic Epidemic Start PointEpidemic End Point

  6. I&W : SARS • Un-translated Chinese media document: “school”+”factory” “closings” • Telecommunication data: anomalies • Economic: trade restrictions

  7. “Factors of Disease Emergence” apply to all of these! Human Pathogens Zoonotic Pathogens Animal Pathogens Plant Pathogens

  8. Problem Definition • Human medical surveillance focused • Minimal incorporation of I&Ws • Alerting capability: days (mostly) to weeks (rarely) • Limited geographic and topic coverage • Stovepiped systems and mission statements • No interface between I&W alerting and responder community What can IT do to address this problem?

  9. I&W: Anthrax Flu FluFlu Real Anthrax Cases in DC

  10. ISIS Center Laboratory Global Environment Animals, Plants, Water Cyber Space and Human Space Hospital Hospital Home

  11. Project Sentinel Collaboratory • Linking Medical Information Systems of two different hospitals • Public Health, Disease Surveillance and Geographic Information System • Data Visualization and Medical Weather Service • Federated Organizations and Biometrics • Grid Service for Data Sharing and Interoperability • Information Assurance

  12. Partial IT for Project Sentinel Collaboratory ISIS Biodefense

  13. Infection Number Vs, Infection Rate • GIS Visualization: choropleth map • Difficult to assess DC 3

  14. GIS Visualization: “Micromap” • DC ranked 6th in the nation for WNV infection risk 3

  15. Ward 3 identified as area of highest risk for infection Medical Vanguard 3

  16. Graded Alerting- Each I&W has a different level of significance that implies imminence of threat to humans Allowing more time to prepare for potential epidemics 3

  17. Building a Wider Net • Data from 100 Emergency Rooms • ScriptRx • Transportation Pattern Data • Electronic Communication Data • 2000 Pet Clinics: Diagnostic Data • Food Security Related Data

  18. I&Ws and Current US Biosurveillance Strategy Bioevent Diagnosis Post-Event Pre-Event Peri-Event I&Ws Current US Strategy I&Ws for bioevents offer weeks to months of lead time alerting… I&Ws prime the response system

  19. Animal Situation Room Medical Health Situation IT Infrastructure Integrated Biodefense Situation Room C. Peck

  20. Technical Objectives • Demonstrate the use of • Scalability • Network Aware • Geographic Information System • To support applications in • Public Health • Medical Decision Making • Large Scale Health Emergency
