  1. Transitional Case Management Services Inside the Los Angeles County Jail

When It Began • Transitional Case Management Services existed at the Los Angeles County Jail for several years prior to 2004 • Then, in 2004 there was a National Demonstration Project, Funded by Centers for Disease Control and in collaboration with and Los Angeles County Office of AIDS Programs and Policy (currently DHSP) and the Los Angeles County Sheriff’s Department. • It brought together Community Based Organizations, Law Enforcement, and Healthcare Providers to demonstrate how services could be provided in incarcerated settings

  3. What is Transitional Case Management? The Transitional Case Management Team Works collaboratively with the LASD Medical Services Infection Control Unit. We provide services to incarcerated individuals who are living with HIV/AIDS and transitioning back to the community. Transitional Services include: * Complete psychosocial assessment * Individual care plan development * Referrals to housing, community case management, medical, mental health as well as drug treatment.

  5. Client Assignment Process Client assignments are distributed to each TCM on a daily basis. The acting HIV Program Manager, Diana Perez, with the support of Kim Bui, DHSP collects information from multiple sources in an effort to identify all known HIV+ inmates within the LAC jail system. • Ex: Inmate disclosure, Prop 96 list, HIV consent forms, jails epidemiology list

Client Intake and Comprehensive Assessment Client intake determines if a client is eligible for transitional case management using the Transitional Case Management baseline intake assessment tool.Comprehensive assessment is a cooperative and interactive face-to-face interview process during which the client’s strengths, needs, andavailable resources are identified and evaluated using the transitional case management comprehensive assessment tool.

  7. Individual Release Plan The Individual Release Plan (IRP) Determines the course of action for the provision of transitional case management services in conjunction with the client. IRP is developed during the intake process or shortly thereafter.

  8. Linkage to Care After the client is released from Jail, the assigned TCM attempts to verify whether the client followed through with IRP. Calls are made to Medical Providers & Community Case Managers in an effort to ensure that clients are linked to care.
