Our Mission Statement: • To bring the best medical care and value to patients through innovation and a highly qualified network of providers in the most cost-effective way • “PATIENT'S VALUES FIRST” HIE Benefits: • Improve healthcare delivery and information gathering • Real-time integration between different EHR, EMR for an instantaneous patient care and improve patient’s safety • Support providers/caregivers with tools to provide patient with the best quality care

Terminology • HIE: Health Information Exchange • HL7: Used in United States as a standard messaging protocol to transfer electronic data between different healthcare systems • ADT Alert: Admit-Discharge-Transfer Alert in HL7 format • ORU: Observation Results in HL7 format for lab results, imaging study reports, EKG, patient’s conditions, etc. • TCM: Transitional Care Management • CCM: Chronic Care Management

MD24 HIE Roadmap • HIE integration internally between our Telemedicine, Smart Lab, and EHR for our patients • Apply to our partners and vendors • Plan to growth together with state HIE (Colorado expansion) • Machine Learning Risk-Level Algorithms based on ICD-10, SNOMED, and patient’s conditions

  7. ‹#› The Stack • HIE Project • PHP backend to get HL7 data from sftp site (AzHEC provides) • PHP parsing the HL7 data on Linux data warehouse • Call API from data warehouse to analyze in Intermediate database then display on BOX(Back-office system for automatic care plan (or other system) • Optional (use Mirth Tools) to setup channel and pull data back • Machine Learning Risk Eval • Host on H2O - http://www.h2o.ai/h2o/ Machine learning will use R/Python • More Data means better outcome
