Case Management Outcome Desired: Provide individualized, integrated, interagency and intergovernmental support for the wounded, injured, ill service member and his/her family throughout the process of treatment, rehabilitation and renewal to: minimize fragmentation of federal services, improve the coordination of medical and rehabilitative care, and ensure access to all needed resources. Charter: Build an integrated, comprehensive DOD/VA program to provide timely, proactive, longitudinal, seamless, collaborative, coordination of quality healthcare and social services to meet specific needs of individual service member and their families in a manner that promotes positive outcomes and quality of life. Key Players: Dr. Davis, Dr. Smith, Dr. Huycke, OSD(HA), Military Departments (e.g medical case managers, family support case managers Severely Injured Marines and Sailors programs), Department of VA (e.g. Seamless Transition Program), Department of Labor, State Rehabilitative Agencies, Civilian Experts.
