Review your prioritized areas for Management of Transition Practices and School in Community and develop a plan for moving forward. Remember resources are available in the Transition toolkit to assist in the planning process. 3. Identify resources that may be missing from the toolkit.   5. The actual set of transition practices enacted with a given family or classroom must be based on the needs and strengths of that child. Positive school connections can be built when schools reflect on the strengths of the child. 5
