  2. Key Findings With the Physician Outreach Initiative • The success of this initiative has been the result of a multipronged approach. • Visits, binders, mail outs, workshops, single telephone number access, single person lead • Local physicians and primary health care providers recognize gaps in their knowledge base in the areas of child developmental screening, assessment and referral for intervention services.

  3. Key Findings With the Physician Outreach Initiative • Local physicians and primary health care providers are receptive to receiving education and resources from the Middlesex London Health Unit and other key community stakeholders in regard to child developmental screening, assessment, community supports and intervention services. • Visits, binder, educational mail outs, workshops, etc.

  4. Key Findings With the Physician Outreach Initiative • Local physicians, primary care providers, and key community stakeholders are committed and willing to partner in an effort to provide enhanced educational opportunities and local services for primary health care providers, community service workers, and families with young children 0 – 4 years of age. • Establishment of the Physician Champion Group, commitment to workshops, etc.

  5. Key Findings With the Physician Outreach Initiative • This initiative has resulted in increased interdisciplinary awareness among service providers and health professionals within the London and Middlesex Community. • Because the services offered by MLHU are constantly changing, it is important to maintain an ongoing, current means of communication with the physician practices to keep them up to date on the various MLHU programs and resources.

  6. Key Findings With the Physician Outreach Initiative • Information for primary health care providers needs to be succinct, evidence based, current and pertinent to the physician’s practice. • Working together we can provide a more seamless process for families with young children and expedite intervention services when needed.
