  3. Coordination and Integration • 2008-9: 4 pilot states work on integration • Others begin talking about integration, mostly coordination • Potential chronic disease “block grant” funding • 2011-12: All states and territories receive coordinated chronic disease funding

Interest in Integration • To meet chronic disease needs without specific programmatic funding • Sustainability of most important initiatives • Increased focus on physical activity and nutrition cut across all disease prevention and management • Focus on policy, environment and systems change • Fiscal constraints require efficiency and effectiveness

  5. Integration is… • A mindset • Organizational change • A whole that is greater than the sum of the parts • A means to an end, not the end in itself • Can’t “sort of” integrate A deliberate, systematic process to redesign what work we do and how we do our work.

  6. Integration Process • What are we integrating to achieve? • The integrated planning process • How do we organize to be successful? • Organizational alignment • How do we transition and change the way we do our business? • Change management

  11. Partner Engagement • External leadership team • Input throughout this conference • Plan as a foundation for dialogue and engagement on an ongoing basis

  12. Focused Input • What are the critical chronic disease trends, issues, needs? • What is the role of the State Chronic Disease and Health Promotion Program? • What (1 thing) must be achieved in the next 4 years relative to chronic disease in NV?
