  1. Informatics competencies in nursing EDUCATION ATI National Nurse Educator Summit April 13-16, 2014 Orlando, Florida Tami H. Wyatt, PhD, RN, CNE, ANEF

  2. Disclosure The presenter is a Co-Inventor of an educational electronic health record that was purchased by Lippincott, Williams & Wilkins in December 2010. The presenter remains a consultant on the modifications and revisions of the EHR owned by LWW. This presentation will discuss how EHRS might be used to meet informatics competencies including rationale and strategies, not particular products.

  3. Informatics Competencies Objectives 1. Discuss informatics related competencies as required by: a. Boards of Nursing b. CCNE c. ACEN d. QSEN 2. Discuss strategies to meet informatics competencies.

  4. Technology & Users Today Review changes in technology related to nursing over the past 50 years.

  5. Technology at Warp Speed Review changes in technology related to nursing over the past 50 years.

  6. Informatics? Definitions “Combining nursing science, information management science, and computer science to manage and process nursing data, information, and knowledge to deliver quality care to the public.” (HRSA, 2008). “Facilitates the integration of data, information, and knowledge to support patients, nurses, and other providers in their decision-making in all roles and settings.” ANA (2008).

  7. IOM identified 5 competencies Institute of Medicine 5 Competencies • patient centered care • interdisciplinary team • evidence-based practice • quality improvement • informatics

  8. And what does this mean for nursing? Future of Nursing Leading Change Nursing must educate future professionals to deliver patient centered care as members of an interprofessional team, emphasizing evidence based practice, quality improvement approaches, and informatics (IOM, 2003b).

  9. How does “informatics” translate to nursing education? “Informatics” has been defined by governing bodies. • National Council of State Boards of Nursing • AACN– Commission of Collegiate Nursing Education • ACEN- Accreditation Commission for Education in Nursing (Previously NLN-AC) • “Most nursing programs are incorporating [evidence based nursing] into their curriculums and the Quality and Safety Education for Nurses (QSEN) national initiative.” www.ncsbn.org

  10. 2020 Collaborative Model NCSBN AACN ACEN

  11. NCSBN & Informatics Standards vary from state to state but core standards exists as related to health information technology and informatics. Transition to Practice Model …nurses are expected to meet the Quality and Safety Education for Nurses (QSEN) competencies… Evidence Based Nursing & Education • Integrating evidence into the program • Practice* • Education* • Policy • Regulation • Faculty Regulations * Reference Informatics in standards

  12. AACN-CCNE Essentials for Nursing Education as related to health information technology or informatics. Essential IV: Information Management and Application of Patient Care Technology …graduates must have competence in the use of information technology systems*. …Specific introductory level nursing informatics competencies* include the ability to use selected applications in a comfortable and knowledgeable way …cost effectiveness and safety depend on evidence based practice, outcomes research, interprofessional care coordination, and electronic health records*, all of which involve information management and technology. * Reference Informatics in standards

  13. ACEN Informatics is a common theme in 3 of the 6 integrated concepts Integrated Concepts • Context & Environment* • Knowledge & Science* • Personal & professional development • Quality & Safety* • Relationship centered care • Teamwork * Reference Informatics in standards

  14. QSEN Informatics integrated into knowledge, skills, and attitude competencies Pre-Licensure Competencies • Patient-centered Care • Teamwork and Collaboration • Evidence-based Practice (EBP) • Quality Improvement (QI) • Safety • Informatics* Central to the QSEN competencies because none are possible without informatics competencies.

  15. Informatics Tools Tools that aid in decision making to support care • Is Informatics synonymous with EHR/EMR? data repository aids in decision making • Tools to aid decisions • Spreadsheets • Data Reports • Cardiac Rhythms • Ultrasonography • Handheld lab monitors • Glucometer • Automated vital sign machines • Respiratory-heart rate-BP monitors

  16. Informatics Strategies

  17. Techniques & Strategies • Present case studies in a classroom setting and locate evidence thru knowledgebase and EHR. • Examine data in an EHR and determine ways data can be queried to answer clinical questions • Determine care recommendations based on evidence in Knowledgebase • Manipulate spreadsheet of data to answer staffing and census questions.

  18. Techniques & Strategies • Trend patient data to anticipate outcomes in spreadsheet, heart monitor tracings, EHR. • Conduct quality improvement studies using data from varied sources. • Outbreak of CLABSI and trend common factors using reports. (query reports) • Trend previous patient data and identify quality and safety issues

  19. How would/do you promote informatics competencies in your curriculum... • …In the simulation and lab setting? • …In the clinical setting? • …As an independent online assignment? • …In group work? • …In the classroom setting? • …In a research course? • …In a management and leadership course?
