BACKGROUND ( 2 ) • Medical testing results including: • Tuberculin skin test Radiographic findings (Chest x-ray, MRI, CT) Bacteriology Pathology or biopsy Drug susceptibility • Date of suspected/verified diagnosis and site of disease • Treatment type and start date • Directly Observed Therapy (DOT) or self-administered
