Hoover Hall Fire- Summary • A student relatively new (January 2017) moved two plastic bags of Lithium waste from a glove box to a fume hood • While cleaning a crucible with HNO3 a beaker spilled and the acid came into contact with one Lithium waste bag • Lithium began to smoke • Student closed the fume hood and began to look for a fire extinguisher (was unable to find one) • Student exited into the hall, found a colleague, and informed him of the fire in the lab

  3. Summary- Continued • Fire alarm was pulled • Building was evacuated • No injuries but the sprinklers did go off • Most of the damage was smoke and water • Specifically water damage to the floor below.

  5. Ames Lab Take Away • Please do not work alone • Revisit (and document) site specific training • Double check labels of materials • Correct fire suppression systems • This is the fourth fire in Hoover since 2012 • Unknown number of near misses or smaller unreported fires.

  8. Notice of violation (NOV) • July 15, 2017 Los Alamos National Laboratory (LANL) received NOV for New Mexico Hazardous Waste Act and Hazardous Waste Management Regulations • Proposed civil penalty $49,630.00

  9. List of Violations • Failure to make a hazardous waste determination at various LANL sites • Failure to keep hazardous waste containers closed • Failure to mark containers as hazardous waste at Satellite Accumulation Areas (SAA) • Failure to provide hazardous waste training for employees signing hazardous waste manifests and handling hazardous waste

  10. Violations (Cont) • Failure to maintain personnel training records • Failure to maintain a facility to minimize potential for fire or unplanned sudden or non sudden release of hazardous waste • Failure to properly dispose of characteristic hazardous waste • Failure to label and demonstrate the length of time universal waste lamps were accumulated
