The HCSIS and IM4Q Connection • Approximately 2 years ago, HCSIS began to incorporate IM4Q into its design. • The most important reason for the switch to an electronic system was so that all IM4Q related information could be shared uniformly across all invested parties. (local program, AE, SCO.etc)

  4. The HCSIS and IM4Q Connection • During the early stages of the IM4Q electronic transition, many SCO’s were overwhelmed. Thus creating the “stressed out Supports Coordinator”.

  5. The HCSIS and IM4Q Connection The “stress” was short lived however, due to the ability to use IM4Q consideration reports in HCSIS which makes the tracking of IM4Q considerations much easier.

  6. IM4Q Reports in HCSIS • At the Supports Coordination Organization level, there are 6 IM4Q reports that we have access to. IM4Q Aggregate Consideration Theme Report IM4Q Aggregate Signs of Quality Theme Report IM4Q Consideration Annual Aggregate Report IM4Q Consumer Pre Survey Detail Report(s): IM4Q Consideration AE Report Summary Report(s): IM4Q Consideration Follow Up Report

  7. IM4Q Reports in HCSIS These reports provide information that can be beneficial to the SCO. However, if tracking considerations for your particular SCO has been a challenge, the most useful report to utilize is the IM4Q Consideration AE Report. Let’s take a closer look into this report…..

  8. IM4Q Consideration AE Report The IM4Q Consideration AE report is helpful to SCO’s because the report will show all of the IM4Q considerations your organization currently has in HCSIS. SCO’s can use this report to help generate an EXCEL spreadsheet that can help with an agency’s internal tracking of considerations. There are a few important things to remember….

  9. Important things to remember for the IM4Q Consideration AE Report…. This report is a “request "report. You will need to allow HCSIS 24 hours to complete this request.

  10. Important things to remember for the IM4Q Consideration AE Report…. After the 24 hour period has lapsed, you can go and retrieve this report from your reports inbox.

  11. Important things to remember for the IM4Q Consideration AE Report…. • This report can be requested daily by your SCO. It’s very important to run the report often because information can and will change in this report as you use this report to submit your agencies considerations to your local Administrative Entity. Run this report often!!!!!

  12. Inside the AE Consideration Report Important: To optimize the use of this report, always open this report in Excel. This will allow an easy transition in creating your own internal tracking spreadsheet for your agency.

  13. Here’s an example of an internal tracking spreadsheet taken directly from the IM4Q Considerations AE Report, but with some personalized additions to make tracking considerations for your SCO.

At the Administrative Entity level, there are 10 IM4Q reports that we have access to: IM4Q Aggregate Consideration Theme Report IM4Q Aggregate Response Report IM4Q Aggregate Signs of Quality Theme Report IM4Q Consideration Annual Aggregate Report IM4Q Consumer Pre Survey IM4Q Survey Status Report IM4Q Survey Status Report for State Centers Detail Report(s): IM4Q Consideration AE Report IM4Q Consideration Report with Interviewers Summary Report(s): IM4Q Consideration Follow Up Report IM4Q Reports in HCSIS

IM4Q Consideration AE Report • Report provides information by county/joinder • SCO • SC • MCI • individual name • consideration ID# • consideration narrative • date of revision • AE/SCO response • The AE uses this report to see what considerations have been entered by the Local IM4Q program as well which ones have been addressed by the SCO. • The AE can see which considerations have been sent back to SCO and not responded to. Let’s take a look

  18. One of the draw backs of this report is that it does not include the date of the consideration being entered. Not allowing the AE to track that considerations are being address according to the County Closing the Loop policy. • To address this, the local program sends us a bi-weekly report of all considerations that have been entered, along with individuals interviewed that did not have a consideration. Let’s take a look at the Local IM4Q Spreadsheet

  19. Utilizing both reports, I have created my own spreadsheet.

Aggregate Consideration Primary Theme Report • Identifies “Status” of each consideration to help track and determine any required action • Identifies consideration as “open/closed” • Providers numerical total of primary and secondary consideration themes, as well as major concerns, thus helping identify any patterns/trends in considerations prompting quality initiatives

  21. The Annual Aggregate Report • Identifies primary considerations with a break-out with all secondary themes • Identifies numerical totals by individual, considerations and average days for SCO to address considerations • Identifies baseline data for comparisons by primary and secondary themes

IM4Q Reports in HCSIS • At the Local Program level, there are 10 IM4Q reports that we have access to (same as the AE): IM4Q Aggregate Consideration Theme Report IM4Q Aggregate Response Report IM4Q Aggregate Signs of Quality Theme Report IM4Q Consideration Annual Aggregate Report IM4Q Consumer Pre Survey IM4Q Survey Status Report IM4Q Survey Status Report for State Centers Detail Report(s): IM4Q Consideration AE Report IM4Q Consideration Report with Interviewers Summary Report(s): IM4Q Consideration Follow Up Report

  26. Considerations Report with Interviewers (continued) It’s helpful to download the "CSV" style report and the PDF report. Use the PDF report to match the headings on the CSV report. The CSV report is more useful because you can easily sort the data.

  27. Our IM4Q Team Members find this information very helpful. The Team Members can see that, as a result of their work, individuals’ lives are changed for the better.
