What Is The EAP? • EAP is a confidential counseling and problem-solving resource designed to help you identify personal issues and work to resolve their difficulties.
Employee Assistance Is: • Problem assessment • Solution-focused counseling • Referral • Information & resource provider • Consultation
Confidential Services • If you contact Performance Plus, no one will know about it unless you tell them. • Information will not be released to anyone unless you give written authorization, by court order, or as required by law. • Participation in the EAP will not jeopardize job security or future promotional opportunities.
Who Is Eligible? • EAP services are available for employees and their dependent family members. • Adult children who live with you are covered until age 21.
What Does It Cost? • Assessment and referral services are free to you. • Your employer covers the cost of your EAP services. • If additional services are needed, the counselor can help you explore referral options in line with your insurance benefits and budget.
Our EAP Counselors Performance Plus Counselors have a minimum of five years counseling experience following state licensure in their areas of specialty. These specialties include: • Social Workers • Licensed Professional Counselors • Psychologists • Marriage and Family Therapists
