Vision, Mission, Philosophy School Mission The mission of Cordova Middle School is to provide a safe and challenging climate that will enable all students to experience high quality instruction and master grade level expectations while becoming lifelong learners. Vision Cordova Middle School will be the premier school within the district attracting a diverse student population, employing effective teachers, developing student leaders, overall exemplifying educational excellence.   5. PBIS/RTI Purpose Statement The purpose of implementing Positive Behavior and Interventions and Supports at CMOS is to foster and promote a safe and positive school environment that enhances social, emotional, and academic success for all students.

  6. Attendance Data Trends • Attendance has continued to increase from 2015- 2018. • We met our attendance goal for 2017- 2018 school year. • Attendance: 96.5% (2015- 2016) 97.1% (2016- 2017) 98.2 % (2017-2018) Revised 8/18

Behavior/Discipline Data Trends •ISS: Decreased 26% from 2015 to 2018. •OSS: Decreased 17% from 15-16 to 16-17 school year, but increased by 5% from the 16-17 to 1-18. •Referrals: Decreased by more than 43% from 2015 to 2018. •Expulsions: Increased by 1 from 15-16 to 16- 17, but decreased by 1 from 16-17 to 17-18.

  11. Monitoring Process The PBIS Team will meet every 20 days to monitor discipline data and the team will strategize and execute a targeted plan of action to deal with discipline issues. The team will use the data by issuing behavior incentives and monitor the number of decreases in office referrals/suspensions and also the number of students involved/participating in school-wide behavior incentives. The team will ensure that teachers are following all discipline protocols and will reiterate procedures to faculty/staff every 20 days. The Team will communicate all information/data to staff/parents/students during Faculty Meetings, Parent meetings, Parent Forums, PLCs, and Class meetings. Revised 8/17

  20. ARRIVAL PROCEDURES School day begins at 7:15 a.m. and ends at 2:15 p.m. Students are to be in their homerooms at 7:15 am! Students are considered late to school after 7:20. • Upon arrival, students are expected to cross streets only at designated crossings. The crosswalk is located on Sanga directly in front of the school. Before care students enter through the front entrance and report to the cafeteria. • Walkers and car riders will enter the front entrance at 6:45a for breakfast. • If students are not eating breakfast they will wait in front of the school until the bell rings at 7:00a. • At 7:00a, ALL students will report to their homeroom class. Students who arrive after 7:20a will report to Ms. O’Bannan in the foyer or the office to receive a tardy slip. • Car riders’ drop-off will be in the front lot each morning and departure will be in the front drive only. • After 2:30, students will wait inside the foyer area for pickup. Car riders will use front entrance doors as they exit to be picked up.

DISMISSAL PROCEDURES School day begins at 7:15 a.m. and ends at 2:15 p.m. • Students will wait to be dismissed at the close of the announcements or by their teacher. • Students will use Voice Level 1 when they enter the hallway. • Students will walk on the right side of the hallway until they exit the building. • Walkers will be dismissed from their grade level hallways. • Students are expected to cross streets only at designated crossings. The crosswalk is located on Sanga directly in front of the school. • At 2:15 car riders will use front entrance doors as they exit to be picked up. • Students are to load cars at the crosswalk area with the faculty monitor. • After 2:30p car riders who remain at the school will report to the main entrance foyer area until their ride arrives

ASSEMBLY PROCEDURES • Students are to enter the assembly at a Voice Level 1 and in a single file line • Students are to sit in their assigned homeroom sections • Teachers are to monitor student behavior during the assembly • During the assembly students are at a Voice Level 0 until the conclusion of the assembly. • Students will follow directions first time given. • Students are to give positive feed • Students are to sit facing forward, keeping their hands, feet and other objects to themselves. • At the closing of the assembly, students will follow instructions from staff, remain at a Voice Level 1, and in a single file line.

  23. CLASSROOM PROCEDURES • Students will use Voice Level 1 for group work and Voice level 0 when instructed. • Students will ask for permission to leave seat. • Students will raise their hand and wait to be called on before talking out in class. • Student will turn in homework as designated by the teacher • Students will respect the property of others, keep their hands, feet and other objects to themselves. • Students are to sit in their designated seats facing forward with their feet underneath the table • Students will follow directions first time given. • Teachers will have a Make-up work/grade recovery policy • Communication with parents (School Messenger, Parent Connect, phone calls, e-mail, Remind, school website.) • Students will rotate to lockers at scheduled times.   24. Hall Pass Procedures • Students will receive a hall pass upon gaining permission to leave the classroom • Students will return hall pass to the classroom teacher as soon as they return to class • Students without a hall pass will escorted back to class.

CAFETERIA PROCEDURES • Students will enter the lunchroom at Voice Level 0. • Students will be seated by next class period after lunch. • Students will use Voice Level 1 when instructed by monitors. • Students ask for permission BEFORE leaving their seat. • Students will line up in one behind the other when instructed to line up. • Students will follow directions first time given. • Students will get bathroom pass after they are granted permission to use the restroom. • When instructed, students will return to a Voice Level 0 at the time of lunch dismissal. • Students are to dispose of trash and push their chairs under the table before lining up to leave the cafeteria. • Teachers will pick up students.

  26. RESTROOM PROCEDURES • Students must have a Hall Pass or accompanied by a teacher . • Students will use Voice Level 1 inside restroom. • Students are to flush the toilet after using. • Students will wash hands with soap • Students are to respect the privacy of others. • Students will keep hands, feet, and other objects to themselves. • Students will dispose of trash in the trash can • Exit the restroom at a Voice Level 0 and return to class promptly.

  27. HALLWAY/LOCKER PROCEDURES • Students will use Voice Level 1 unless otherwise instructed. • Students will walk on the right side of the hallway. • Students are to maintain their personal space at all times. • Students should always walk in the hallway. • Students will help other students with lockers when needed. • Students are to have permission to go to lockers or be escorted by teacher. • Students will report any inappropriate behavior to teachers
