You are encouraged to customizetheseaccording to your needs. It would probablybebest not to read themout loud but to use your own words. • WHY PARTICIPATE INTHE DIABETES PREVENTION PROGRAM? • Diabetes is aserious problem for AmericanIndians and AlaskaNatives (AIANs). • AIANs are2-3 times morelikelyto get diabetes than others in the United States. • Research shows thatyoucan decreaseyourrisk ofgetting diabetes throughmodest lifestyle changes suchas increased physicalactivityand alittle weight loss. • WHAT DOES THE DIABETES PREVENTION PROGRAMOFFER? • DiabetesPrevention Education • Learn howto make changes in lifestylebehaviorssuch as increased physical activityand healthier foodchoices (lowerfat and fewercalories). These changes will helpyou to lose weight. • Presented in a class setting/group format with aproven curriculum for16 sessions over 16-24 weeks. • Individualized Coachingand Support • You will meet separatelywith aLifestyleCoach onceamonth to discuss howthings are going and to helpyou individualizeyourprogram to meetyourpersonal needs and style. • Additional Group Classes such as cooking andexerciseclasses. • WHYWOULD YOUWANT TO PARTICIPATE? • To help reduceyour chances ofgetting diabetes. • To receiveprofessional advicethrough Diabetes Prevention Education. • Healthierlifestyleforalongerlife– Whatyou learn at the Diabetes Prevention classes--physical activityand healthier food choices (lessfat and fewercalories)--will leadyou to ahealthier lifestyle. • Theprogram(classesand individual sessions with lifestylecoaches, fitness experts, and nutritionists)is worth over$5000 in care. • WHAT DO YOUHAVE TO DO? • Attend 16 classes overaperiod of16-24 weeks. • Begin to build exerciseintoyourlife,workingyour wayup to 150 minutes per week (or10,000 steps perday). • Meet with aLifestyleCoach onceamonth. • Get aphysical exam before and aftertheclassesand then onceeachyearto monitoryour change in health and glucoselevels. • Afteroneyearin theprogram, between the annual physical exams, haveyourglucoselevel measured. • CompletetheComprehensiveParticipant Questionnaire whenyou start theclasses, whenyou completethe classes, andannuallyevery yearthatyou remain in theprogram. • Sign aHIPAAB form beforeyou start theprogram to showthatyou agreeto allowsomeofyour personal information, withoutyourname, dateofbirth orotheridentifiers,to beshared with the Coordinating Center.
