Objectives of the Medicare Local • Improve the patient journey through developing integrated and coordinated services • Provide support to clinicians and service providers to improve patient care • Identify health needs of the local areas and develop locally focused and responsive services • Successfully implement primary health care initiatives and programs • Be efficient and accountable with strong governance and effective management

Aims of FMPML PURPOSE: • high quality, primary health care that is accessible and coordinated STRUCTURE: • independent, constituted body that is a service system manager, a coordinator of partnerships ACTIVITIES: • Demonstrable, targeted impact • Based on shared population health data (evidence) • Partner co-designed • Build on existing work/organisations • Robust evaluation

Strategic Priorities of FMPML • Provide support to clinicians and health providers • Afterhours primary health care • Mental health • Under-serviced populations • Addressing local needs • Ageing (dementia) • Enable the aged to remain independent for as long as possible • Vulnerable children & families • Early intervention creates better health • Proactive health / living well • Preventative health strategies
