Change to Partner Name If name of current partner changes it is partner’s responsibility to inform UoS in the following way: No change in site of delivery or of ownership Change of ownership or takeover • Written confirmation required • stating the reason for the change • in name and that the provision is • continuing with the same resources. ADC consideration. Approved? MAR inform Ac Services and issue revised MoA/CPA. • If change in name involves • a change in the delivery • site for the provision • it should be dealt with • through process D3.
