HSE Near-miss Reporting System Procedure Document Number: KOC.GE.047 • Purpose / Scope:This procedure describes a system for Near-miss reporting, investigation, root cause analysis and time bound corrective measures including follow up action as well as sharing of lesson learnt. • The following are the key actions/deliverables indicated out of this procedure for the purpose of induction only, enabling contractor to estimate basic resource requirements (it does not mean that the other actions/deliverables are not applicable): • All and any employees shall report Near-miss case that have occurred in their area • Shall have near-miss reporting system or can utilize KOC Near-miss system (i.e. • KOC Near-mix reporting boxes, near-miss reporting cards/forms etc.) • Shall have system in place for investigation and time bound implementation plan of • the corrective/preventive measures and follow-up for action close-out • Shall have in place near-miss training/awareness program for all employees and • the program shall be implemented • Shall have in place a near-miss reporting incentive program to encourage employees • to report near-misses
