Aim

By 2010, all disabled young people in Birmingham should have a person centred transition plan using the Transition Pathway

  • Multi Agency funded training
  • Wide range of participants – schools, health and social care workers, Connexions, 3rd Sector agencies
  • 59 schools trained to date including all special schools
  • Targeted approach for mainstream schools based on % of statemented pupils

  • SENAS accept Transition Pathway Plans and documentation
  • 200+ plans submitted to SENAS
  • Links with CAF for monitoring and analysis
  • Follow up to training – now includes briefing on post 16 and post 19 training opportunities
  • IAG – leaflets and events

Independent evaluation with Wave 1

Support and commitment from senior managers

Named person with responsibility, authority and time to oversee implementation

Action Plan has been developed and implemented

TP has been incorporated into the curriculum – often via PHSE and citizenship
