As the use of PET-CT in the staging and monitoring of oncologic disease continues to expand, familiarity with patterns of bowel FDG uptake is essential and can help determine the need for and the relative urgency of further testing. • Even in the absence of a CT abnormality, incidental focal bowel uptake should not be taken lightly. • The incidental finding of focal FDG uptake in the bowel justifies further investigation (endoscopic or surgical procedure) of these foci and should not be dismissed as physiologic uptake. • A substantial number of these patients may have malignant (unsuspected bowel metastases) or premalignant lesions (adenoma).
