  5. Confirm you have all of your providers listed then click on “OK”

  7. Type in the number of well child visits you had and the number of books distributed between Jan. 1, 2013 and June 30, 2013.

  8. Type in the name and email address of the person responsible for ensuring that new providers are trained in the program

  12. The #1 spot should say MDAAP. If you have received donations/used books, you can put their names on the other lines. (For examples, staff, Girl Scouts, etc.) Check any that you provide and then give the number of literacy referrals you made. It is OK to put in 0.

  13. This can be left blank if you do not have a volunteer program. Here you can write and comments/ suggestions on the program. It is always nice and useful to hear how parents/children are receiving the program.

  14. This information is also in your application. Please select this last option. This way any extra books can go to practices not supported by the grant. Thank you.

  15. Enter the contact person and address to which all books should be shipped.

  16. Please sign in both places. Hit “Submit” and you are done!
