Eligibility Criteria Family Services All carers of someone with a mental illness can access
Eligibility CriteriaDay and Education Programs • Diagnosis of Mental Illness • Aged between 18 and 64 • Access to transport to attend
Eligibility CriteriaDoorway • Diagnosis of mental illness • Complex needs • Homeless or risk of homelessness • Referral from LRH
Preferred Referral Process • Self Referral • Formal Referral: • Though mental health service, psychologist, psychiatrist, GP’s, Centrelink • Completion of SCoTT • S2S
