Introduction • Annual Justification – The program physician’s renewed prescription for the patient to continue with maintenance treatment. • Regulatory and policy documentation • Regulatory requirement – Title 9, Division 4, Chapter 4, Subchapter 5, Article 5, section 10410. Scheduled Termination of Maintenance Treatment. • Aegis form: Annual and Two-Year Justification for Maintenance Treatment – Doc.1116.06.00

  3. Introduction cont. • You will learn: • State regulatory timelines • Aegis Medical System procedure timelines • Responsible staff • Correct completion of the form.

  4. Vocabulary • Annual Justification – The completion of Doc. 1116.06.00 to satisfy California state regulatory requirements. Completed by Primary Counselor, reviewed and co-signed by Clinic Manager and Program Physician. • Annual physical – aka Comprehensive Medical Examination. Often confused with Annual Justification. Completed by program physician seeing patient for comprehensive physical or consultation appointment.

  5. Vocabulary cont. • Continuous treatment date (CTD) – The date the patient first entered Maintenance treatment. There must not be any break in treatment. This may or may not be the admission date for the current program. • Break in treatment – A period of time when a patient is not enrolled in a narcotic treatment program. • Completion date – The date the program physician reviewed and signed the form.

  6. Timeline • The form is completed annually from the patient’s continuous treatment date. • State regulation specify 2 years from admission date then annually there after. • Aegis policy is to justify the patient’s continued treatment annually. • The form is completed up to 60 days prior to the anniversary date as indicated by program physician’s dated signature .

  7. Control Log and Charting • The completion date is logged in the annual justification log. • The form is filed in the patient record. • When thinning a patient record, the most recent annual justification following the last state inspections must remain in the active patient record.
