Definition of gonorrhoea ALLOPATHIC • STD caused by Neisseria gonorrhoea. • Transmitted via direct contact with infected epithelium. • Those at risk: Sexually active, between 15 and 30 years old, multiple sex partners, urban areas, homosexual men. TIBB • STD caused by an excess of heat & moisture brought about by infection with Neiserria gonorrhoea. • Sanguinous imbalance. • Pathway 1 • Also at risk: Sanguinous individuals, especially with Bilious subdominance.

  4. Signs and symptoms

  5. men • Creamy or green, puss-like urethral discharge, with inflammation of the meatus • Dysuria, urinary frequency and urgency • Rarely, abscesses of Tyson's and Littre's glands, periurethral abscesses, or infection of Cowper's glands, the prostate, or the seminal vesicles.

  6. women • Menorrhagia and intermenstrual bleeding • Creamy or green, pus-like or bloody vaginal discharge • Dysuria, urinary frequency and urgency with purulent urethral discharge  • Dyspareunia • PID- Salpingitis, pelvic peritonitis, pelvic abscesses • Fitz-Hugh-Curtis syndrome- Perihepatitis that causes right upper quadrant abdominal pain, fever, nausea, and vomiting.

  7. Men and women • Rectal infection- Creamy, pus-like discharge; constipation; bleeding; pruritis ani. • Pharyngeal infection- Sore throat. • Disseminated gonococcal infection (DGI) (arthritis-dermatitis syndrome) caused by bacteraemia- fever, migratory pain, joint swelling, and pustular skin lesions. Skin lesions occur typically on the arms or legs, have a red base, and are small, slightly painful, and often pustular.

  8. neonates Gonorrheal ophthalmia: • Acute purulent conjunctivitis 2 to 5 days after birth • Severe eyelid oedema • Chemosis • Profuse purulent exudate • If untreated- corneal ulcerations and blindness Suspect sexual abuse in a child with any gonococcal symptoms!

  9. diagnosis Genital fluids, blood, or joint fluids tested by: • Gram stain • Culture • Nucleic acid-based test Thereafter: • Serologic test for syphilis • Screening test for chlamydia

  10. Treatment and management

  11. counselling • Commend patient for seeking early treatment. • Reassure that there is a cure. • Inform patient of his/her responsibility to be compliant with treatment and to practice safe sex thereafter.

  12. Lifestyle advice Diet: • Avoid H+M food. • Avoid alcohol, fizzy drinks & processed food. • Increase C+D, as well as C+M & D+H food. • 8 glasses of water every day. • Fresh fruit, vegetabes & high fibre foods. Other: • Manage or avoid stress & anxiety. • Slow & deep breathing exercises. • Avoid strenuous physical activity. • 8 hours of sleep every night. • Promote elimination.

  13. Regimental therapies& medication • Massage: Cooling oils • Cupping: LE13 & LE14 • Septoguard Tablets (D+H): 2 TDS • Renotone Syrup (M+H): 10ml TDS • Livotibb Tablets (M+H): 1 TDS • Vgel (females): As needed

  14. education Correct use of barrier contraceptive methods: • Demonstration • Hand out • Stress that OCPs & IUDs do not protect from STDs.

  15. Alternative therapies • Lactobacillus acidophilus or live-culture yogurts • Zinc, multivitamins, mineral complexes, vitamin C, and garlic (Alliumsativum) to improve immune function. • Kelp contains vitamins and minerals for a faster recovery. • Calendula (Calendula officinalis), myrrh (Commiphoramolmol), and thuja (Thujaoccidentalis) to reduce discharge and inflammation. • Fasting or a juice fast. • Acupressure & acupuncture therapy.

  16. prognosis Longstanding infection: • PID- Infertility & ectopic pregnancy risk • Sterility in men • Perihepatitis • DGI- Joint pathology & skin lesions Early treatment cures the patient of the infection!
