  1. Referral from Client to Scottish centre for Healthy working lives (take demographics and pass file on to Forth Valley Working Health Services (Case Manager) • Practitioner Referral to Forth Valley working health services Case Manager by GP, A&E, OPD or other health professional on behalf of client. Referral from other professional body i.e. Council Services, S/W, Employability, Job Centre plus, Disability employment advisor, should be made as assisted referral through Centre for Healthy Working Lives free phone number(0800 0192 211) Working health Service Forth Valley – Patient Pathway Forth Valley Working Health Services Case Manager contacts client within 2 working days and conducts telephone assessment, offers advice and/or offers client appointment for face to face assessment within 5 working days Forth Valley Working health services Case Manager/Vocational Rehabilitation nurse meets with client, full assessment of required intervention carried out and planned return to work contract discussed with client, consent for GP and employer contact obtained Condition Management provided by FV WHS Condition/Case Manager using a bio-psychosocial model to deconstruct many of the clients problems and assist clients to manage their condition giving appropriate advice and considering access to the relevant therapy. Condition/Case Manager has contact throughout and links with the following services and offers additional support as required Rapid Referral Non – Rapid Referral Physiotherapy Intervention If musculoskeletal condition/problem referral by Case Manager to forth valley physiotherapy Working Health services link with rapid response to referral Counselling Intervention If specialist counselling requiredWorking Health services Case Manager referral to forth Valley Counselling Service with rapid response to referral Signpost to National Services and help lines including but not exhaustive, Back Care, Breathing Space, Samaritans, SADA, National Self Harm Network, Anxiety UK, Moodjuice, Living life to the full, DASH, Bipolar Fellowship, Salvation Army courses, BEAT, APNI, SANDS, CRUSE Signpost to other Forth Valley services including but not exhaustive, FDAMH, SDAMH, SAMH, ASC+ GO Forth, Signpost Citizens Advice Beaurau, Headway FV, Council Debt and Benefit Advice Service Onward referral to other FVHB services including Mental Health Resource Centre, CADS, Epilepsy Connections, MS Nurse, Stroke Nurse, Cardiac Rehabilitation, Pain Services, Diabetes Specialist Nurse Referral to General Practitioner if change in treatment required or more intensive treatment required i.e. if condition remains or becomes severe and enduring Occupational Therapy Intervention If specialist intervention required to enable return to work Working health services Case Manager will refer to Occupational Therapy WHS Link within FV Possibility of In work Assessment and Liaison with employer Exercise Referral by Working Health services Case Manager to either Active Forth, Stirling Active or Clackmannanshire Sports Council. Vocational rehabilitation/Case Manager maintains contact with the client throughout and when the client has completed with required intervention, re-assesses the client, now considering a return to work the possibility of engaging Healthy working lives and Scottish Centre healthy working lives for both employee and employer Condition/Case Manager reports back to SCHWL with outcome, also sends report to GP and employer with clients consent and follows up with patient at 3 monthly intervals up to 1 year post completion.
