  3. WHAT DOES YOUR INSURANCE COORDINATOR DO? • - enrolls you with UHC • - explains benefits and responsibilities • - issues with United Health Care

  4. HOW DO YOU PAY FOR YOUR INSURANCE? Your insurance charge is automatically put on your Bursar bill.

  5. WHEN DOES COVERAGE BEGIN? -- You are automatically enrolled each term if you have an assistantship. The insurance starts with the beginning of each term. -- You will receive your insurance card in 10 to 14 days

  6. WAIVERS If you have an assistantship and you have other health coverage you may waive out of the insurance. The waiver form and instructions on how to waive out of the insurance is on the graduate website. All waivers must be turned by the 15th day of class.

  7. WHAT IS THE PROCEDURE IF YOU ARE SICK? You must go to the Auburn University Medical Clinic (AUMC) first!! Referrals from the AUMC are required!! (p. 3)

  8. WHEN DO INOTGO TO THE AUMC? • - Medical emergency • - AUMC is closed • - Break or vacation periods • -- More than 50 miles from AU

  9. WHERE DO I GO WHEN THE AUMC IS CLOSED? -- Auburn Urgent Care -- Emergency room

  10. WHAT WILL MY INSURANCE PLAN PAY FOR ?? What are my “benefits?” -- Most medical expenses -- Maximum $250,000 for each sickness or injury -- Most expenses at 100% with PREFERRED PROVIDERS

  11. WHAT AND WHO ARE “PREFERRED PROVIDERS?” These are health care providers near and around Auburn University (within 50 miles) that have established standard billing charges with United Health Care. UHC will pay 100% of these charges.

  12. WHAT HAPPENS IF I DON’T USE A PREFERRED PROVIDER? Charges paid at 80% of what is considered “usual and customary rates” and 100% after $5,000

  13. WHERE CAN I FIND A LIST OF PREFERRED PROVIDERS? -- Website -- UHC -- Insurance Coordinator

  14. WHAT IS MY DEDUCTIBLE AND COPAY? -- $200 per person per year deductible -- $25 Copay per outpatient physician visit -- $75 Copay for emergency room visit
