  1. CQI at Floyd • Opportunity for improvement: Decrease # of pts w/ A1c >9% • Collecting data: chart audit for NCQA recognition • Analyzing data: 19 of 75 charts reviewed (25%) had A1c >9%. Goal < 15% per NCQA

CQI at Floyd • Choosing New Approach: Referral into Real World Diabetes class: a 2 part series class (classes 1 wk apart w/ specific homework tasks) • Implementation: MD refers to class, pt attends, track A1c at next MD visit. • Evaluate: Average A1c prior to class 10.3%, after class 9.4%. • Maintenance: Continue to offer class

  3. CQI at Floyd • This project almost overwhelming due to many issues seen that contributes to pts’ A1c: pts did not show up for class, A1c better but still not at goal, psychosocial issues. Committee did brain storm and working on things but realizing staff and funds needed.

  4. CQI at Floyd • Opportunity for Improvement: Increase # of participants in support group • Collecting data: Reviewed # of people who signed in at support group • Analyzing data: Average of 6 participants per support group

CQI at Floyd • Choosing New Approach: Offer support group during the day instead of the evening • Implementation: Beginning in 2012, support group will be offered from 10-11a.m. third Tuesday of every month • Evaluation: Continue to collect data and evaluate 4th quarter 2012 • Maintenance: To be determined
