Stages of EHR Maturity and Patient Collaboration • The new ‘patient team’ business models, like ACOs, will require that EHRs mature into real-time care coordination and collaboration platforms that can help move organizations • Care coordination and collaboration aren’t just about adding patient messaging and simple health records sharing • Managing that new data to help hospitals and physicians across patient care teams better comprehend what is happening inside the patient so that they can actually improve health outcomes Mark Renfro, HTH

Stages of EHR Maturity Patient Collaboration Independent care (Stage 1-MU) The first stage is where individuals or separate organizations provide much of the patient care using electronic health care solutions Hometown Health and their affiliated hospital have been deeply involved with this process by installing the clinical information suites in order to meet Stage 1 of CMS “Meaningful Use” Mark Renfro, HTH

Stages of EHR Maturity Patient Collaboration Connected Care (Stage 2-MU) In the second stage providers begin to share enough information to begin basic patient care collaboration Examples between the Hospital and Clinic, then later between other levels of provider care (with regional facilities and specialists using tools like Georgia Healthcare Information Network Mark Renfro, HTH

Stage 2-MU • Stage 2 criteria - places an emphasis on health information exchange between providers to improve care coordination for patients • Key New Stage 2 objectives: • Interoperability • Patient Portal - Record electronic notes in patient records • Physician, Care Providers and Hospital Medical Record Exchange • Requiring interfaces in order to coordination with other entities and State Sponsored Programs • Use cost savings approach by sharing services with other HIT providers Mark Renfro, HTH

Stages of EHR Maturity Patient Collaboration Coordinated Care The third stage begins when enough patient data is connected, so that it allows multiple organizations to coordinate their care between each other through case tracking and management such as identifying health risks. The Patient-Centered Medical Home (PCMH) is an approach to providing comprehensive primary care for children, youth and adults. Mark Renfro, HTH

How does FCC/USAC Health Care Fund fit with MU Broadband Infrastructure • The Healthcare Connect Fund (HCF) Program is the newest component of the Rural Health Care Program. • Eligible expenses related to broadband connectivity New eligible services include: • Cloud-based connectivity services and point-to-multipoint • Network equipment necessary to make broadband service functional • Connections to, and equipment located at, data centers and off-site administrative offices Mark Renfro, HTH
