General information about the LVN/Paramedic to ADN transition • One academic year (three semesters) that rotate around the summer semester For Spring 2020, begin in mid-January and end in mid-May 2021 • Earn an Associate’s of Applied Science in Nursing degree • Prepares students to sit for National Council Licensure Examination (NCLEX-RN) • Curriculum includes a combination of class lectures, simulation experience, and five clinical rotations • Full Acceptance into the program is contingent upon submission of all required immunizations, and clearance on the physical exam and drug screen

Approvals and accreditation • The Texas Higher Education Coordinating Board (THECB) and the Texas Board of Nursing approves the ADN Transition Program. • The program is accredited by the Accreditation Commission for Education in Nursing (ACEN) and regulated by the Texas Board of Nursing. • We are a technical program, and subject to change at any time to meet the requirements of our approvals and accreditation. **Note: Effective January 1, 1996, a person who has been convicted of a felony may be disqualified for obtaining licensure as a Registered Nurse. After turning in your completed application, you may be contacted for further information by email.

  5. Application process- two steps Step 1- You must first apply on ApplyTexas on San Jacinto College’s website and be accepted to San Jacinto College. (If you are a prior student, and out of classes for two semesters or more, you will need to complete this step to update your records.) Step 2- After acceptance to San Jacinto College, you will need to submit an application packet to be considered for acceptance into the LVN/Paramedic to ADN Transition Program.

Application process- san jacinto college requirements • Submit an online application to San Jacinto College at www.applytexas.org/adappc/gen/c_start.WBX. • Applicants must submit all previous and current official college transcripts to Admissions and request transcript evaluation. Foreign transcripts will need to be evaluated. • Submit an official San Jacinto College transcript if prior or current student and copies of other transcripts from other schools with ADN Transition application. • Direct inquiries to Advisors in Admissions Office in the Welcome Center, Bldg. S-6.

Requirements for admissions • State of Texas Licensed Vocational Nurse or Licensed Paramedic in good standing • Provide a copy of your current Vocational Nursing License or Paramedic License • CPR Certification- LVN’s must provide proof of certification as BLS Healthcare Provider through American Heart Association www.heart.org or 1-800-242-8721 -OR- • ACLS Certification- Paramedics must provide proof of this certification • Have not failed out of a registered nursing program within 1 year of the application deadline

Requirements for admissions • Successful completion of Science courses with a grade of “C” or better within 5 years of the first day of class. • Example: For Spring 2020, classes cannot be older than Spring 2015 • Successful completion of all the pre-requisite courses with a 2.6 or higher GPA • Minimum overall course work GPA of 2.5, no exceptions to GPA requirements • LPN-ADN Entrance Exam or EMS-ADN Entrance Exam • Completed TITERS for immunizations with required documentation

Application submission • Applications will be accepted in the ADN Transition office, S-1.253b on Tuesday, Wednesday, and Thursdays from 9:00am-11:00am and 2:00pm-4:00pm during the application period. (exact days posted on the website) • If you need to come outside of the available times please email adn.transition@sjcd.edu to schedule an appointment. • All required documents must be submitted as a complete packet to the ADN transition program office in the application period specified. • Application is posted online at www.sanjac.edu/adn-transition.

  10. Pre-requisite courses

  11. Proof of pre-requisite completion • A printed copy from My San Jac GPS (Graduation Plan Strategy) of your degree evaluation in your application packet. (If San Jacinto College accepts the courses taken at another institution as credit earned, the nursing department will accept the course as credit.) • If the course is not accepted as credit earned, you can request a Course Substitution from the Department Chair of the course subject in question. It is the applicant’s responsibility to provide documentation to the Department Chair of each course in question. For more information on course substitution, please contact Admissions. REMINDER: The ADN Transition program is a technical program, subject to change at any time!

LVN-ADN Entrance hesi exam • 75-item comprehensive exam used for Licensed Vocational Nurses seeking entrance into an Associate Degree Nursing program, limited to 1 hour and 53 minutes • Measures competencies across LPN/VN content areas to identify strengths and weaknesses, and the possible need for remediation prior to the entrance into an ADN program. • Approximate cost $56.00, limit one exam at San Jacinto College • Elsevier has recommended study resources of NCLEX-PN style questions to assist in preparation. EMS-ADN Entrance hesi exam • 50-item comprehensive exam used for Licensed Paramedics seeking entrance into an Associate Degree Nursing program, limited to 1 hour and 13 minutes • Measures competencies across basic nursing content areas to identify strengths and weaknesses, and the possible need for remediation prior to the entrance into an ADN program. • Approximate cost $37.00, limit one exam at San Jacinto College • Elsevier has recommended study resources that assisted in passing your Paramedic licensing exam, or link shown in picture: https://www.mhprofessional.com/9780071849029-usa-mcgraw-hill-educations-emt-paramedic-exam-review-third-edition-group

HesiENTRAnce exam - how to register • If you are ready to test, you can register today for the next test on July 8, 2019. • You can email adn.transition@sjcd.edu when you are ready to test for the next available test date. Subject Line should specify the HESI Exam you desire to register for. • You will be allowed to register to test with us only one time. No minimum score required, points are based upon composite score (see Rubric on website) • Approximately two days before the exam you will receive registration and payment instructions. • HESI Exams must be completed prior to submission of your ADN transition application.

HESI entrance Exam - day of exam • Arrive 10-15 minutes in advance for instructions • Bring photo identification • Know your Evolve username and password • Instructions will be provided regarding how to access the exam • Calculators, headsets, pencils, and scratch paper available * * Must arrive on time. We will not accept late students!

Immunization requirements • Titers showing immunity required for:MMR, Varicella, HEP B • TD/TDAP vaccine (within 10 years) • TB test (within 1 year) or clear chest X-ray (within 2 years) • Seasonal flu vaccine (current within 1 year and then continuous in program) - Proof of completed immunizations are required at the time of application - Purchase of the Castlebranch document management system and Drug Screen package will be required after acceptance, and is approximately $60

  17. Rubric • Admissions rubric may be found on the website at: • www.sanjac.edu/adn-transition • Selection is based upon numerical ranking on the rubric. INCOMPLETE APPLICATION PACKETS WLL NOT BE ACCEPTED!!!
