Life threatening clinical syndrome of cardiovascular collapse characterized by • Hypotension • Hypoperfusion
Primary shock – transient vasovagal attack , reduced venous return to heart , peripheral pooling of blood trauma, severe pain, fear, sorrow • Secondary shock – true progressive reduced oxygen supply at tissue level
Clinical presentation • Uncounciousness , syncope • Weakness , sinking sensation • Pale and cold extremities • Tachypneoa • Tachycardia • Feeble pulse • Subnormal temperature • Reduced blood pressure
Etiological classification • Hypovolemic shock • Cardiogenic shock • Septic shock • Other types
Pathogenesis • Hypotension • Tissue Anoxia • Tissue injury and release of inflammatory mediators
Stages of shock • Compensated / Reversible / Non progressive shock • Progressive Decompensated shock • Irreversible Decompensated shock
Morphological changes IN ORGANS • HYPOXIC ENCEPHALOPATHY • SUBENDOCARDIAL HEMORRHAGE • ACUTE TUBULAR NECROSIS KIDNEY • ARDS LUNG • HEMORRHAGIC GASTROENTERPATHY • FOCAL NECROSIS LIVER • ADRENAL HEMORRHAGIC NECROSIS
LIFE THREATENING COMPLICATIONS • Acute respiratory distress syndrome( ARDS) • Disseminated intravascular coagulation(DIC) • Acute renal failure (ARF) • Multiple organ dysfunction syndrome (MODS)
